Measure B and Vehicle Registration Fee Program Compliance Report Forms
For end-of-year compliance, Measure B and Vehicle Registration Fee fund recipients are required to submit to the Alameda CTC one electronic and one hard copy version of an Audited Financial Statement and an Annual Compliance Report for the respective programs.
Recipients of Vehicle Registration Fee (VRF) Funds must complete a separate Audited Financial Statement and an Annual VRF Compliance Report.

Measure B / Vehicle Registration Fee Compliance Reporting Forms
The FY 12-13 Measure B/VRF Compliance Reporting Forms include two attachments. (1) MS WORD Form, and (2) a Table 1-3 Attachment. The MS WORD Form is generic to all agencies for the Measure B/VRF Programs. whereas Table 1-3 Attachment is specific to each agency.
Measure B/VRF Compliance Revisions
As of February 2013, Alameda CTC Staff and the Citizens Watchdog Committee have completed their review of the submitted Audited Financial Statements and Program Compliance Reports. A Request for Information to confirm compliance has been sent to the jurisdictions (as applicable). Jurisdictions are expected to respond by no later than Friday, February 21, 2014.

Send an email with the completed MS WORD Form(s), MS EXCEL Tables 1-3 attachment(s), and additional attachments to firstname.lastname@example.org and post-mark hard copies before the due date.
